Fairview Health Services is seeking a Same Day Nurse Practitioner to join our primary care practice at M Health Fairview Clinic Princeton. Join our thriving multi-specialty practice with onsite access to a robust group of specialty providers and services.
Practice Details:
- 17 provider primary care practice, including 6 FM with OB physicians.
- Patient encounters average 16-20 patients per day.
- 34 patient contact hours per week.
- Openings: 0.8 to 1.0 FTE - Flexible schedule
- As a Pioneer Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care.
- Fully integrated Electronic Medical Record (EPIC)
Responsibilities
- Assess health status for specific patient populations using advanced practice knowledge and skills.
- Obtains relevant health and medical history.
- Performs a physical examination based on age and medical history. Performs/orders preventative and diagnostic procedures based on the patient's age and history.
- Identifies health and medical risk factors.
- Identifies acute and chronic health problems.
- Appropriately orders and accurately interprets diagnostic results.
- Develops and implements an individualized plan of care.
- Establishes practice agreements with collaborating physician.
- Collaborates with health care team members in developing realistic care expectations.
- Determines effectiveness of treatment plan with documentation of patient care outcomes.
- Revises and modifies the care plan as needed to achieve health and medical goals.
- Refers care to other health care professionals and community agencies as needed.
- Prescribes pharmacologic (including controlled substances) and non-pharmacologic agents.
- Communicates effectively with patients and family for understanding of diagnosis, treatment plan, and need for follow-up care.
- Provides education to patients, families or caregivers as needed.
- Documents according to established guidelines to facilitate ongoing quality care in a timely manner.
- Responds in a timely manner to phone calls regarding patient needs.
- Contributes to effective and efficient practice management policies and practices that support patient access to medical care.
- Participates in access planning for applicable area or population.
- Supports appropriate billing for professional service through timely documentation and maintaining adequate knowledge of FVC billing policies and procedures.
- Adheres to all relevant policies, procedures, and practice guidelines.
- Assists with the preparation for JCAHO and other regulatory surveys.
- Manages patient care through appropriate use of available resources.
- Appropriately prescribes and orders ancillary services.
- Utilizes Fairview referral network appropriately.
- Manages chronic health/disease conditions.
- Provides leadership for clinical and support staff.
- Serves as a resource to support staff, enhancing quality of care.
- Participates in the training, orientation, and evaluation of clinical and support staff.
- Participates in appropriate clinical investigation relevant to specific patient population.
- Responds to nursing staff educational needs in formal and informal venue demonstrating nursing procedures, charting procedures, and reviewing pathophysiology of disease and new treatment modalities.
- Serves as consultant and role model for nursing staff.
- Maintains competence and professional development. Engages in continuing education.
- Is active in professional and other appropriate organizations.
- Demonstrates behaviors that support performance improvement activities.
- Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served.

Required Qualifications
- Graduate of an accredited school of nursing and formal nurse practitioner training program.
- BC/BE through the AANP or ANCC
- Licensed or ability to obtain APRN license with the Minnesota Board of Nursing
- Ability to obtain and /or maintain DEA certification for the State of Minnesota
- Meet threshold criteria/qualifications for Credentialing and Privileges
Preferred Qualifications
- Minimum two years of experience.
Qualifications:
$119,246 - $132,496 Annual *based on full time (1.0 FTE)
