Kent Hospital, part of the Care New England Health System in Rhode Island, is seeking a physician to join our innovative and successful Hospital at Home (HaH) team. Kents HaH program, which had its start in 2022, has served over 1000 patients, delivering high quality hospital-level care to patients in their homes. This care delivery model is part of a growing national trend and has been shown to improve patient outcomes, reduce complications, lower hospital readmission rates, and enhance patient satisfaction - all while easing the strain on hospital capacity.

Kent HaH patients receive two in person nursing visits daily, one physician visit daily (typically in-person, but occasionally virtual), are monitored 24/7 via telemetry via a light-weight biopatch sensor, and have access to a range of inpatient services such as labwork, mobile diagnostic imaging, PT/OT, and physician consults.

This position is well suited to a physician who excels in patient-centered care, builds strong relationships with patients and families, enjoys working in an innovative practice environment, and thrives in a collaborative and multidisciplinary team.

Principal clinical responsibilities:

  • Serve as the attending physician for a 10 bed HaH unit (average daily census of 8 patients), providing direct oversight and coordination of patients acute care plan.
  • Manage a variety of acute inpatient conditions, including but not limited to pneumonia, heart failure, COPD exacerbations, urinary tract infections and cellulitis.
  • Conduct home visits either in person or virtually for patient evaluations, utilizing remote patient monitoring technology and virtual care tools to enhance patient management.
  • Help facilitate admissions of new patients to the program, either directly from Kents emergency department or as transfers from the inpatient units.
  • Work in close collaboration with a multidisciplinary team, including nurses, advanced practice providers, case managers, social workers, pharmacists, and EMT/paramedics.

Qualifications

  • BE/BC Internal Medicine or Family Medicine.
  • Hospitalist experience is preferred (or equivalent experience managing patients in an acute inpatient setting)
  • Rhode Island medical license (or eligibility to obtain one)
  • Proficiency or interest in digital and mobile health, healthcare innovation
  • Exceptional intrapersonal and communication skills and track record of providing patient- centered care.
