Job Description
Under general supervision of the CARE Center Medical Director and in collaboration with the Director of Population Health, responsible for diagnosing and treating patients subject to the individuals written agreement approved by the Maryland Joint Committee on Nurse Practitioners. Provides continuity of patient care by facilitating and coordinating communication between the health care team, patient and patients family. Coordinates care to ensure quality, effective care and patient safety. Serves as link to integrating relevant services both internal and community-based consistent with the Institute for Healthcare Improvements Triple Aim of Better Health, Better Care, and Lower Cost.
Qualifications
Education & Training: Master of Science degree with completion of a Nurse Practitioner program. Registered and currently recorded with the Maryland Board of Examiners of Nurses and Certified as a Nurse Practitioner in the State of Maryland. (Copy of written agreement with physician signed by the physician and nurse practitioner and written approval by the Joint Committee for Nurse Practitioners pursuant to COMAR 10.27.07.06A.) DEA and CDS eligible. BCLS required; ACLS preferred.
Work Orientation & Experience: Requires a minimum of three years of acute care experience or other accepted related experience as appropriate. Demonstrated ability to promote professionalism through involvement in professional organizations, teaching, research, publishing, and/or certification in area of specialty. Demonstrate superior knowledge of the multidisciplinary care management process and continuum of care, emerging care issues and trends including application of new technology, and outcomes management.
