We are seeking a board certified Family Medicine Nurse Practitioner (NP) or Physician Assistant (PA)for our UHS Primary Care- Upper Front Street location. This is a busy, Family Medicine out-patient practice in a well-established office.Position is 100% outpatient. Office hours Monday- Friday 8:00 a.m. - 5:00 p.m. with rotating call. Great opportunity to join ateam of 4APPs and 5 Physicians.

Salary Range: $126,300 - $148,600 plus quality and productivity bonuses

Education/Experience

Minimum Required:

  • Master of Science in Nursing (MSN) degree or PA degree.
  • Possession of an NPI (National Provider Identification) number, as evidenced by the receipt of a copy of the notification assigning the number


License/Certification

Minimum Required:

  • Current NYS Registered Professional Nurse License
  • American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners (AANP) certification, or PA board certification.
  • Current NYS license
  • Documentation of current CPR certification required within one (1) week of hire, with maintenance of current CPR certification during employment
  • Certified Prescriptive Privilege in NYS
