Qualifications:

  • Education: Master’s or Doctoral degree in Nursing (FNP or AGNP focus).
  • Licensure: Active New York State NP License; Board Certification required.
  • Experience: New graduates are welcome; prior experience in geriatrics or nursing homes is a plus.
  • Technical Skills: Proficiency in EHR systems, chronic disease management, and infection control protocols.

Key Responsibilities:

  • Conduct comprehensive patient assessments, including medical history reviews and physical exams.
  • Develop and update individualized care plans for residents with acute/chronic conditions.
  • Monitor and manage infections by adhering to facility protocols.
  • Educate patients and families on disease management, prevention, and wellness.
  • Collaborate with physicians, nurses, and therapists to ensure coordinated care.
  • Maintain accurate documentation of treatments, progress, and care plans.
