Position Overview:

Greenlife Healthcare Staffing is seeking a Maryland-licensed Physician to provide on-site and on-call medical services for intellectually disabled adults (18+) residing at the Potomac Center (ICF/ID, up to 62 residents) and SETT (secure forensic facility, up to 32 residents) in Hagerstown, Maryland. The physician provides routine and urgent care, annual evaluations, and supports compliance with ICF/ID licensure and federal standards.

Qualifications:

  • Licensed Physician (MD or DO):
    • Active, unrestricted license to practice medicine from the Maryland Board of Physicians.
    • Experience and comfort treating adults with intellectual disabilities and co-occurring mental/behavioral health conditions, including maladaptive and self-injurious behaviors.
  • Hospital Privileges: Admitting privileges (or ability to obtain) at Meritus Medical Center in Hagerstown, MD.
  • Background & Health Clearance:
    • Willingness to complete a CJIS State and federal criminal background check (fingerprinting).
    • Provide a recent medical exam (within the last 6 months) documenting:
      • General health and past communicable disease history.
      • Current TB test meeting licensure/ICF/ID standards.
      • Letter of commitment to provide services for the full contract duration.
  • Professional Requirements:
    • Curriculum vitae with at least 3 recent professional references.
    • National Provider Identifier (NPI) and CMS Certification Number (CCN).
    • Maintain all required certifications and remain in good standing (no disqualifying disciplinary actions); promptly disclose any new investigations, actions, or legal proceedings.
  • Ability to:
    • Work independently and collaborate closely with an interdisciplinary team (nursing, behavioral health, administration).
    • Manage complex medical needs in a secure, structured, habilitation/forensic setting.
  • Please Provide the following:

    • Brief note confirming or describing:
    • Your ICF/ID, developmental disabilities, or forensic/behavioral health experience
    • Your comfort level working with adults with intellectual disabilities and maladaptive behaviors
    • Your current or planned Meritus Medical Center admitting privileges
    • Your general weekly availability for at least two on-site days in Hagerstown, MD
    • (Optional but helpful) Copies of the most recent:
    • TB test result
    • CPR/First Aid/AED certification

Job Responsibilities:

On-Site Services (min. 6 hours/week):

  • Provide medical care to all Potomac Center and SETT residents, including:
    • Comprehensive history and physicals for new admissions (on next scheduled on-site day) and annually thereafter.
    • Annual and as-needed evaluations to certify continued need for ICF/ID-level care.
    • Routine sick calls and episodic care for acute medical conditions.
    • Monthly review of each resident and review/renewal of medications as needed.
  • Develop and document:
    • Physician’s Orders
    • Physician’s Progress Notes
    • Plans of care, consultations, and lab reviews in the medical record.
  • Review 90-day plan-of-care summaries and perform all medical duties needed to meet:
    • ICF/ID licensure standards (OHCQ/MDH)
    • Federal Title XIX requirements
  • Initiate and coordinate medical and surgical consultations and hospital referrals.
  • Provide guidance to nursing and facility staff on medical care policies and resident-specific care.
  • Sign in and out each day on site for hours tracking and invoicing.
  • Comply with:
    • Potomac Center/SETT Contraband & Restricted Items Policy (no personal electronics around residents)
    • Tobacco-free campus policy

On-Call Duties:

  • Provide 24/7 physician on-call coverage for:
    • Emergencies
    • New admissions
    • Urgent clinical questions from facility staff
  • Respond to on-call inquiries within 1 hour.
  • Provide daily on-call availability from 8:00 a.m. to 5:30 p.m. for consultations.
  • Assist with emergency medical placement (e.g., contacting local hospitals, explaining resident needs and required services).
  • Arrange qualified substitute physician coverage (pre-approved by the facility) if unavailable.
