As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APCs, registered dietitians, registered nurses, and registered pharmacists, to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity related conditions as appropriate in a primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health.
 
Schedule: Monday- Friday with flexible scheduling, 32 patient contact hours minimum (+ 8hrs of administrative time).? Hybrid model of care. No scheduled evening/weekend responsibilities outside of phone On-Call 
EMR: Athena 
On-Call: Phone call only, in a rotation for one (1) week at a time. Average call volume - <1 call per WEEK (as of 8.1.24). 24/7/365 physician back up. 
Location: Hybrid in Plano, TX

Responsibilities:

      • Practice exceptional medicine within knownwell 
      • Sees adults for metabolic health, and primary care services as is appropriate in a virtual setting.  This may include sharing a panel with MD/DO + other APCs of the local in-clinic practices. 
      • May see pediatric patients ages 12+ for metabolic health only, based on training, licensure, and competency. 
      • Remote care using audio and visual technology to complete patient examinations, evaluation, assessments, diagnoses and treatment plans.   
      • Embodies the Knownwell mission and values of patient inclusivity in how you deliver care. 
      • Coordinates with the broader care team to ensure the patient has the support they need. 
      • Proficiency with Microsoft Office applications recommended, but not required.  
      • Documents as necessary to ensure accurate billing, including review of remote patient monitoring data. 
      • Communicates with patients in a timely manner through our portal for questions and concerns that have been pre-screened to necessitate a clinician response. 
      • Ensure patients feel knownwell 
      • Ensure your patient panel is open to any individual, regardless of their current health status and comorbidities. 
      • Listen to the aspirations of the patient and ensure care is tailored to the patient’s unique health journey. 
      • Provide an environment in which patients feel comfortable and free from judgment. 
      • Offer exceptional encouragement as patients reach their goals. 
      • Coordinate across the collaborative care team 
      • Escalate any concerns to your dedicated site leadership team. 
      • Provide clinical guidance to medical assistants and nursing teams as assigned.
