Were seeking a passionate and driven Physician Assistant/Nurse Practitioner to join our Lacks Enterprises - Primary Care team in Kentwood, Michigan. This is an exciting opportunity to make a meaningful impact on the health of our community while working in a supportive, mission-driven environment.
Opportunity Details
- Position Type: Full-Time (1.0 FTE)
- Schedule: Monday to Friday, 8:30am to 5pm
- No call, weekends or holidays
- Location: 4365 52nd St SE Kentwood, MI 49512
- Specialty: Occupational Medicine, Sick/Urgent Visits for primary care and Personal Health Assessment reviews
- Additional Details:
- Lacks Primary Care offices are operated by Trinity Health for patients and their families enrolled in the Lacks Medical Plan
- Pharmacy and lab on site for patients

Requirements
- Provider experience required
- APPs: Board certified in specialty prior to employment
- Active Michigan medical license (or ability to obtain)
- DEA license
- ACLS/BLS certification (must be maintained)
Duties & Responsibilities
A typical day may include:
- Providing direct patient care
- Collaborating with multidisciplinary teams
- Participating in teaching and mentoring
- Documenting in Epic EMR
- Engaging in quality improvement initiatives
- Supporting community health efforts
