Adapt Integrated Health Care is looking for a qualified Physician to join their Primary Care team!

The Primary Care Provider is responsible for providing primary care services including assessing, diagnosing, and evaluation of injury and/or illness and ordering routine exams as well as treatment of acute and chronic health problems.  They are strongly committed to and promote a strong belief in Adapts philosophy, purpose and ideals.  

Qualifications:

  • Unrestricted Medical, PA, or NP license required
  • Board Certified preferred
  • Extensive experience working in an integrated healthcare setting.

Primary Responsibilities:

  • Performs history taking, physical examination, diagnosis, and therapeutic treatment planning within their scope of practice.
  • Reviews and assesses the patient medical history, including health habits and disease history. Includes basic principles of healthy lifestyle and health risk reduction with patient encounter, orders diagnostic testing as indicated.
  • Interprets results of diagnostic testing and plans appropriate follow-up with individual patients, with consideration given for their physical emotional and financial resources.
  • Utilizes evidence-based practices when ordering testing, interpreting results, diagnosing, and tracking patient conditions. 
  • Makes referrals to specialists and community resources as necessary.
  • Oversees activities of team to ensure consistent and coordinated patient care.
  • Maintains a complete and legible medical record, to include timely documentation of each patient visit.
  • Identifies resources available within the community to assist patients with their health care needs.
  • Plans and provides routine immunizations.
  • Prescribes medications according to prescriptive privileges. Administers medications and injections accordingly. Responds to requests for medication refills in a timely manner.
  • Counsels patients regarding general physical and emotional concerns.
  • Provides patient education through handouts and discussion of patient concerns during office visits. Instructs patient/family regarding medication and treatment directions.
  • Provides consultation by telephone or in person to patients and medical personnel.
