We are seeking a dedicated and compassionate Family Practice Nurse Practitioner to provide vital health assessments and support to patients across Iowa, including Linn, Polk, Black Hawk, and Scott counties. Required Skills:

  • Valid Iowa Medical License with current certification
  • Proven experience in family practice, including pediatrics and adults
  • Exceptional communication and interpersonal skills
  • Ability to manage a flexible schedule and adapt to telehealth and home visits
  • Proficiency in electronic health records (EHR) and documentation
  • Comfortable conducting assessments, risk evaluations, and chronic condition reviews
  • Reliable transportation and willingness to travel within assigned counties

Nice to Have Skills:
  • Experience with Medicaid enrollment and credentialing
  • Familiarity with in-home healthcare protocols
  • Bilingual capabilities (English and Spanish or other languages)
  • Prior telehealth delivery experience
  • Knowledge of Medicare/Medicaid policies and documentation standards

Preferred Education and Experience:
  • Master's or Doctoral level degree in Nursing (MSN, DNP)
  • Minimum of 2 years of primary care experience
  • Certification as a Family Nurse Practitioner (FNP) with active licensure in Iowa

Other Requirements:
  • Enrolled and credentialed in Medicard
  • Ability to work Monday through Friday, 8 am to 5 pm, with scheduled 1-hour lunch breaks
  • Must complete initial orientation and ongoing education modules within scheduled downtime
  • Reimbursement for travel and mileage at IRS standard rates
  • Two to three weeks' scheduling notice, with flexibility for both full-time and part-time commitments
  • Ability to conduct approximately 5-6 assessments daily, utilizing telehealth and in-home visits

Compensation for this role is $95.48 per hour, with comprehensive benefits including weekly pay via direct deposit, medical coverage, referral bonuses, paid sick time, and licensure/certification reimbursements. Full-day pay is provided regardless of patient volume, contingent upon entering availability into Salesforce.
