Position Overview:
We are currently seeking a Registered Nurse (Nurse Surveyor) to support the NYS Department of Healths Center for Home and Community-Based Services by conducting surveillance, quality reviews, and compliance evaluations of licensed and certified home care providers. The role focuses on ensuring compliance with New York State laws, regulations, and codes governing home care agencies, hospices, adult care facilities, and related programs. This position requires strong clinical judgment, regulatory knowledge, and written communication skills.

Qualifications:
- Education: Associate's Degree in Nursing or higher.
- Required:
- Registered Professional Nurse (RN) license.
- Current registration to practice nursing in New York State.
- Preferred:
- Experience in nursing surveillance, utilization review, or regulatory oversight.
- Familiarity with adult care, assisted living, or long-term care regulations.
- Skills:
- Strong clinical nursing judgment.
- Excellent written, verbal, and telephone communication skills.
- Proficiency with Microsoft Office applications.
- Experience with complex data entry and surveillance systems.
- Ability to interpret and apply healthcare regulations and statutes.
- Strong organizational and documentation skills.
- Ability to work independently and as part of a multidisciplinary team.
Job responsibilities:
- Conduct surveillance activities for licensed home care services agencies, certified home health agencies, hospices, and home health aide training programs.
- Perform off-site and on-site post-survey revisits.
- Review, track, and evaluate agency-submitted Plans of Correction.
- Communicate requirements for acceptable Plans of Correction to providers.
- Review and triage new complaint intakes.
- Conduct complaint investigations, including staff interviews, patient interviews, and observations.
- Prepare complaint write-ups, correspondence, and regulatory letters.
- Review applications for new licensed and certified agencies.
- Evaluate facility policies and procedures for regulatory compliance.
- Participate in enforcement actions, including testimony at hearings.
- Prepare reports and documentation using applicable computer systems.
- Participate in meetings, committees, and quality improvement initiatives.
Physical Examination Responsibilities:
- Observe and assess patient care delivery during onsite investigations.
- Evaluate clinical practices for compliance with applicable standards.
- Document findings related to patient care quality and safety.
Medical Oversight Responsibilities:
- Apply nursing expertise to regulatory surveillance and compliance reviews.
- Interpret and apply New York State healthcare laws and regulations.
- Support enforcement actions through accurate clinical documentation.
- Ensure surveillance activities align with Department of Health standards.
