HarmonyCares is seeking a Nurse Practitioner to join our home-based primary care team in the Reading, PA area. This full-time role offers the opportunity to deliver high-quality, relationship-based care to medically complex seniors in their homes.
As a physician-led organization with 30+ years of experience, we focus on outcomes—not volume, giving providers the time and support to practice meaningful medicine.

Role Overview
Provide primary care in a residential setting, including assessment, diagnosis, treatment, documentation, and coordination with the care team.
Qualifications
Required:
• Active, unrestricted NP license (PA) + DEA
• Board Certified/Eligible
• CPR certification
• Ability to travel locally
Preferred:
• Geriatric or complex care experience
• Strong collaboration and communication skills
