Physician - Family Medicine

HarmonyCares is seeking a Physician Lead for our home-based primary care team serving patients along the I-4 corridor (Tampa-Orlando).

This is a full-time, in-home primary care role designed for physicians who want to stay clinically active while also serving as a clinical leadermentoring APPs, influencing care delivery, and driving team performance.

The Role

• ~40-50% protected leadership time
• Balance of clinical care + team leadership
• Smaller patient panel with medically complex seniors
• Typically 6-10 patients/day, 30-60 minute visits
• No clinic setting - care delivered in patients homes

Youll operate within a dyad leadership model, partnering with operations leadership to drive provider performance, patient outcomes, and care quality.

Key Responsibilities

• Supervise and mentor NPs/PAs (collaborating physician role)
• Lead case reviews, ride-alongs, and clinical development
• Conduct chart audits and support quality initiatives (HEDIS)
• Participate in complex patient care and high-risk case discussions
• Partner with leadership on performance, productivity, and care strategy

Qualifications

• MD or DO
• Board Certified or Board Eligible (required)
• Active (or ability to obtain) Florida Medical license
• Valid drivers license
• Comfortable with home-based care delivery
