Nurse Practitioner for Roper St. Francis Physician Partners Primary Care Office located on the campus of a continuing care retirement community, James Island SC. 

NP will also care for rehabilitation patients, as well as long term chronically ill patients.  

Monday thru Friday and Weekend call availability is required.

The practitioner will provide ongoing medical management for primary care clinic located on campus as well as those in the sub- acute rehabilitation facility, long term care and memory care areas.

The APP will work as a member of the interdisciplinary team. The APP will develop updates and evaluate medical treatment plans, medications, and other necessary care. Supervising MD on site.

Minimum Qualifications:

Education: Nurse Practitioner (NP) Masters level program accredited by an appropriate agency recognized by the South Carolina State Board of Medical Examiners (SCSBME).

Experience: 1-2 years of experience as an APP  Adult or  Geriatric with Primary Care, Acute Rehabilitation or Sub-Acute experience preferred.

Licensure/Certification: Must be licensed and/or certified by appropriate agencies as required by state law. Must be licensed by SCSBME. Must have a current American Heart Association BLS and ACLS for Healthcare Provider Card. Other certifications may be required depending on the location of the position.

Knowledge/Skills: Knowledgeable in basic patient care, as well as scope and responsibility of a midlevel provider. Knowledge of the practice to give and evaluate patient care. Knowledge of organizational policies and procedures to administer patient care. Knowledge of common safety hazards and precautions to establish a safe work environment. Skill in applying and modifying the principles, methods and techniques of professional on-going patient care. Skill in identifying problems and recommending solutions. Skill in preparing and maintaining records and writing reports. Skill in establishing and maintaining effective working relationships with patients, hospital, medical staff and the public. Ability to analyze situations and apply critical thinking. Ability to communicate clearly. Ability to react calmly and effectively in emergency situations. Should be able, after training, to be proficient in electronic medical record -EMR EPIC and PointClickCare
