Position Overview:
The Family Medicine Physician will examine, diagnose, and treat patients of all ages for acute injuries, infections, and illnesses, while also providing preventive care and counseling patients and families on acute and chronic conditions. This role offers the opportunity to work within a collaborative, team-based care model focused on high-quality outcomes and patient-centered care.
Practice Details:
- Established primary care practice with a collaborative team-based environment
- Patient encounters average 18 patients per day
- 1.0 FTE = 34 patient contact hours per week
- Call Frequency: Low intensity, phone consultation only with first-tier nurse triage
- Openings: 0.75 to 1.0 FTE (Full Time) flexible scheduling options
- Fully integrated Electronic Medical Record (EPIC)
Responsibilities:
- Perform comprehensive patient evaluations, including history and physical exams
- Develop differential diagnoses and order appropriate diagnostic testing
- Provide preventive, acute, and chronic disease management
- Prescribe medications and administer vaccinations
- Interpret diagnostic results and implement treatment plans
- Refer patients to specialists as appropriate
- Maintain accurate and timely medical documentation
- Participate in cross-coverage and care coordination
- Collaborate within a multidisciplinary care team
- Engage in quality improvement initiatives and system-based practices
- Ensure compliance with all organizational policies, procedures, and regulatory standards
Qualifications:
- Graduate of an accredited ACGME/AOA Family Medicine residency program
- Board Certified or Board Eligible (ABMS or AOA)
- Licensed or ability to obtain licensure through the Minnesota Board of Medical Practice
- Ability to obtain and maintain DEA certification
- Meet credentialing and privileging requirements

$281,854 - $313,172 Annual *based on full time (1.0 FTE)
