How you'll make an impact in this role

AscensionSeton is looking for a PulmonaryHypertensionAPRN or PA to join our AdvancedHeart Failure/transplant/VAD team in an outpatient setting at Ascension Seton Medical Center.

• Join an established team of 11 physicians and APPs
• Practice setting: outpatient
• Employed position within a nonprofit health system

• Lead comprehensive patient assessments by integrating medical history, physical examinations, and diagnostic insights to create a clear and actionable path forward for every patient.

• Design and manage personalized treatment plans for a variety of health conditions, coordinating seamlessly with community resources and specialists to ensure holistic, continuous care.

• Empower patients and their families through dedicated counseling and education, providing them with the knowledge and resources needed to manage their health and prevent future illness.

• Document patient care journeys with precision and care, ensuring that every record reflects a high standard of clinical excellence and supports a safe, efficient healthcare network.

What minimum qualifications you'll need

Licensure / Certification / Registration:

• Nurse Practitioner credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required.
• Licensed Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.
• Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.

Education:

• Master's degree of Nursing required.

What additional preferences you'll need

• Pulmonary Hypertension experience
• NP with acute care certification
