About the Opportunity
The University of Maryland Medical Center Midtown Campus, part of the University of Maryland Medical System, is seeking an experienced and highly skilled Certified Nurse Practitioner (CRNP) to serve as Manager, Clinical Program – Heart Transplant.
This is a unique leadership opportunity within a high-performing academic transplant program known for clinical excellence, innovation, and multidisciplinary collaboration. The role blends advanced clinical practice with program leadership, supporting the full continuum of care for heart transplant patients.
Position Overview
The CRNP Manager of the Heart Transplant Program provides advanced clinical care and operational leadership for complex transplant patients across inpatient and outpatient settings.
This role works closely with cardiology, transplant surgery, critical care, nursing, and allied health teams to ensure safe, high-quality, and patient-centered transplant care, while supporting program growth and regulatory compliance.
Clinical Responsibilities
- Provide advanced practice care for heart transplant patients across the continuum (pre-, peri-, and post-transplant)
- Conduct comprehensive patient assessments, physical examinations, and diagnostic evaluations
- Order, interpret, and manage laboratory and diagnostic testing
- Develop and implement individualized treatment plans in collaboration with transplant physicians
- Manage immunosuppressive therapy and monitor for post-transplant complications
- Ensure continuity of care throughout all phases of transplant management
- Participate in multidisciplinary rounds and care coordination activities
Program Leadership & Coordination
- Collaborate with transplant surgeons, cardiologists, nurses, and multidisciplinary team members
- Participate in case conferences, transplant selection meetings, and care planning sessions
- Support compliance with UNOS, CMS, and institutional transplant regulations
- Contribute to program development, workflow optimization, and strategic growth initiatives
- Assist in development and implementation of clinical protocols and best practices
- Support transplant-related education for staff and trainees
Quality & Outcomes
- Monitor patient outcomes and support continuous quality improvement initiatives
- Assist with transplant metrics tracking, reporting, and regulatory documentation
- Participate in program evaluation, data review, and performance improvement activities
- Promote evidence-based practice and clinical excellence across the program

Qualifications
Required
- Active CRNP licensure in Maryland (or eligibility to obtain)
- Acute Care Nurse Practitioner certification required
- Strong clinical background in cardiology, adult gerontology, critical care, or transplant medicine
- DEA registration (or eligibility)
- Current BLS and ACLS certification
Preferred
- Experience in heart transplant or advanced heart failure care
- Prior leadership or program coordination experience in an advanced practice role
- Knowledge of UNOS and CMS transplant regulations
- Strong communication, leadership, and interdisciplinary collaboration skills
- Ability to manage complex, high-acuity patient populations
