Required Skills:

  • Valid and active Missouri Medical License
  • Enrolled and credentialed in MediCard (confirmation required upon application)
  • Proven experience in family practice, including pediatrics and adult care
  • Strong clinical assessment skills for risk evaluations and chronic condition reviews
  • Excellent communication and patient education abilities
  • Ability to work independently and collaboratively in fast-paced settings
  • Comfortable with telehealth platforms and in-home visits within a 30-mile radius

Nice to Have Skills:
  • Bilingual abilities or additional language proficiency
  • Experience in home health or community-based care
  • Familiarity with Salesforce or similar scheduling and documentation tools
  • Prior experience with pediatric and adult populations in a primary care setting

Preferred Education and Experience:
  • Master’s degree in Nursing (MSN) or higher from an accredited program
  • At least 2 years of clinical experience in family medicine or primary care settings
  • Prior telehealth and/or home health experience

Other Requirements:
  • Willingness to see pediatric and adult patients in a flexible schedule
  • Comfortable performing risk assessments and chronic review care only (no treatment or prescribing)
  • Able to travel within the specified counties for home visits and community engagement
  • Reliable access to necessary equipment provided by the organization, shipped to your location
  • Availability Monday through Friday, 8 am to 5 pm, with scheduled appointments released 2-3 weeks in advance
