Key Responsibilities
- Patient Management: Conduct comprehensive evaluations and follow-up visits for residents in skilled nursing facilities (SNFs).
- Clinical Strategy: Develop, implement, and adjust individualized treatment plans based on patient needs.
- Collaboration: Work closely with physicians, nursing staff, therapists, and facility leadership to ensure cohesive care.
- Documentation: Maintain accurate and timely patient records using Electronic Health Record (EHR) systems.
- Care Coordination: Participate in interdisciplinary care planning meetings to optimize patient outcomes.
Qualifications & Requirements
- Licensure: Valid license as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Florida.
- Skills: Strong clinical assessment skills and a professional demeanor.
- Communication: Excellent interpersonal skills to effectively collaborate with a diverse care team and interact with patients/families.
- Logistics: * Ability to commute daily to Venice, FL (Required).
- Ability to relocate to the area prior to starting work (Required).
- Traits: Attention to detail and the ability to work independently while remaining a team player.
