Our flagship hospital, Baystate Medical Center, is home to the University of Massachusetts Chan Medical School-Baystate. We are seeking an experienced Advanced Practitioner to join our staff.

Position Highlights:

  • Join our vibrant division of Palliative Care that includes 6 Advanced Practice Providers (APPs), 6 physicians, 2 social workers, spiritual support and a highly supportive clinical staff dedicated to providing evidence-based and patient-centered care.
  • Our community palliative care program focuses on providing patient-centered care in a supportive environment to patients and families facing advanced illness and end-of-life challenges.
  • The position involves collaborating closely with an interdisciplinary team consisting of Social Work, Nursing, Spiritual Care Services, Nurse Practitioners and Physicians. Comfort with holding family meetings, discussing goals of care with patients and families, and developing plan of care.
  • Assess and manage symptoms of patients with advanced illnesses in a consultative model with the primary care providers and other specialists involved in the patient’s care. Collaborate with the inpatient palliative care team while assisting with the transition to hospice when appropriate.
  • Candidates will participate in interdisciplinary team meetings that foster collaboration, professional development, and continuous growth.
  • Our Palliative Care team is supported by an established telehealth platform, allowing clinicians to deliver care to patients remotely.
  • This is a .8 FTE community-based position. Schedule offers flexibility providing great work/life balance. Mileage reimbursement and laptop provided. Regions are assigned based on geographic location.

 

Qualifications: BC Nurse Practitioner or Physician Assistant with clinical experience in home care, palliative care, or geriatrics settings.

                                                                          

The compensation ranges in the table below represent the base salaries for all positions at a given grade across the health system. Typically, a new hire can expect a starting salary somewhere in the lower part of the range.  Actual salaries may vary by position and will be determined based on the candidate’s relevant experience. No employee will be paid below the minimum of the range.  Pay ranges are listed as hourly for non-exempt employees and based on assumed full-time commitment for exempt employees.

 

Palliative Care AP: $129,780 - $164,400 for 1.0 FTE, $103,824 – 131,520 for .8 FTE
