PATHS is seeking a Family Nurse Practitioner or Physician Assistant to join our collaborative primary care team in beautiful South Boston, Virginia. Practice evidence-based medicine in a Patient-Centered Medical Home (PCMH) where you'll build meaningful relationships with patients of all ages while enjoying a manageable patient panel and strong physician support.

Schedule

  • Monday-Friday
  • Full-time (40 hours/week)
  • Rotating shared call for evenings and weekends

Responsibilities

You'll provide comprehensive primary care for patients across the lifespan while working alongside an experienced clinical team.

Responsibilities include:

  • Perform patient evaluations, exams, diagnoses and treatment plans
  • Manage acute and chronic medical conditions
  • Conduct preventive care and wellness visits
  • Order and interpret diagnostic testing
  • Prescribe medications and manage refills
  • Coordinate specialty referrals
  • Review test results and communicate with patients
  • Maintain timely documentation within eClinicalWorks
  • Educate patients on disease prevention and healthy lifestyles
  • Participate in quality improvement initiatives
  • Attend provider and staff meetings

Qualifications

Required

  • Licensed Family Nurse Practitioner or Physician Assistant in Virginia
  • Current DEA registration
  • Eligible to bill Medicare, Medicaid and commercial insurance

Preferred

  • Board Certified
  • FQHC experience
  • Experience with eClinicalWorks
