• 2 days per week -- flexible on which days, with hours per day flexible
• 15 - 22 patients per day depending on SNF census
• Skilled nursing facility (SNF) setting
• Regulatory visits required -- MD responsible for every other 60-day visit
• Open to internal medicine or family medicine (adult patients only)
• Hospital privileges required
• 1-year minimum commitment
