Medical Director of Heart Failure at CMC
RWJBarnabas Health is seeking a Medical Director of Heart Failure to provide clinical, strategic, and operational leadership for the Heart Failure Program at Community Medical Center (CMC).
This role ensures delivery of high-quality, guideline-directed, cost-effective heart failure care across the inpatient and outpatient continuum while supporting program growth, quality improvement, and regional integration within the RWJBH Heart & Vascular Service Line.
The Clinical Director partners with hospital leadership, cardiology, nursing, care management, and advanced heart failure programs to strengthen clinical outcomes, optimize transitions of care, and improve patient access to evidence-based therapies.
Key Responsibilities
- Program Leadership and Strategy: Provide clinical leadership for the Heart Failure Program across inpatient and ambulatory settings. Develop and execute program goals aligned with hospital and RWJBH cardiovascular service line priorities. Integrate heart failure services across cardiology, hospital medicine, emergency medicine, case management, and outpatient care. Strengthen referral pathways to advanced heart failure therapies and regional programs while promoting a collaborative, multidisciplinary approach.
- Quality, Outcomes, and Performance Improvement: Lead heart failure quality improvement initiatives focused on readmission reduction, Guideline-Directed Medical Therapy (GDMT) optimization, transition-of-care reliability, and order-set utilization. Monitor program performance using internal dashboards, registries, and national benchmarks. Partner with Quality and Case Management teams to address adverse outcomes and care variation, and support participation in national heart failure registries and accreditation initiatives.
- Clinical Program Development: Develop and maintain standardized heart failure protocols, pathways, and order sets. Support expansion of outpatient heart failure management capabilities, including post-discharge follow-up programs, medication titration workflows, and multidisciplinary clinic models. Evaluate program resource needs including staffing, equipment, and technology, and assist with program budgeting and growth planning.
- Education and Collaboration: Provide clinical guidance to physicians, nurses, and coordinators involved in heart failure care. Support ongoing staff education related to heart failure management and evolving therapies. Maintain strong relationships with referring providers and regional partners while representing CMC in regional and system heart failure initiatives.
- Clinical Responsibilities: Provide cardiology clinical care in ambulatory and/or inpatient settings. Develop an Advanced Practice Provider (APP) based outpatient Heart Failure clinic structure for remote and in-person care. Participate in cardiology call coverage as determined by leadership. Maintain compliance with RWJBH Medical Group policies, Joint Commission standards, and regulatory requirements, completing documentation and medical records in a timely manner while participating in BHMG Medical Group meetings and activities.
Candidate Requirements
- Clinical Competence: Proven ability to provide comprehensive cardiology clinical care in ambulatory and inpatient settings.
- Program Leadership: Demonstrated experience in leading clinical programs, executing program goals, monitoring performance dashboards, and developing standardized protocols or pathways.
- Compliance: Ability to maintain full compliance with RWJBH Medical Group policies, Joint Commission standards, and regulatory requirements.
Compensation & Benefits

The anticipated salary range for this position if hired to work is $500,000 to $650,000 per year. This role is also eligible for additional compensation through performance and productivity-based incentives.
