- Office is located at 2600 N. Detroit St. (S.R.9), LaGrange, IN 46761
- Hours of practice are: Monday - Wednesday 8 a.m. - 5 p.m. / Thursday - Friday 7 a.m. - 4 p.m.
- Team consists of physician and nurse practitioners
- Call schedule: County call rotation only for physicians
- The call center allows for a very light and manageable call with excellent work/life balance.
