HarmonyCares is seeking a Nurse Practitioner to join our home-based primary care team in the Lancaster, PA area. This full-time role offers the opportunity to deliver high-quality, relationship-based care to medically complex seniors in their homes.

Role Overview
Provide primary care in a residential setting, including assessment, diagnosis, treatment, documentation, and coordination with the care team.
Qualifications
Required:
Active, unrestricted NP license (PA) + DEA
Board Certified/Eligible
CPR certification
Ability to travel locally
Preferred:
Geriatric or complex care experience
Strong collaboration and communication skills
