BJC Medical Group is seeking a Board Certified/Board Eligible Internal Medicine or Geriatrics Physician to join West County Medical Associates in a Post-Acute Care role serving patients throughout the West St. Louis County region.

This position is ideal for a physician who enjoys managing medically complex adult and geriatric patients, collaborating across the continuum of care, and making a meaningful impact on patient outcomes during critical transitions from hospital to post-acute settings.
Practice Overview
The Post-Acute Care Physician will provide comprehensive medical care within skilled nursing facilities, rehabilitation centers, assisted living communities, and long-term care facilities. Working alongside an interdisciplinary care team, the physician will focus on improving quality outcomes, reducing readmissions, and ensuring seamless transitions of care.
Key Responsibilities
- Provide admission, follow-up, and transitional care services in post-acute and long-term care settings.
- Manage acute and chronic medical conditions in adult and geriatric populations.
- Coordinate care transitions from Barnes-Jewish West County Hospital, Barnes-Jewish Hospital, and other referring facilities.
- Collaborate with advanced practice providers, nursing staff, therapists, social workers, and specialists.
- Participate in interdisciplinary care conferences and family meetings.
- Conduct goals-of-care and advance care planning discussions.
- Drive quality initiatives focused on reducing avoidable hospitalizations and improving patient outcomes.
- Complete accurate and timely EMR documentation.
- Foster strong relationships with facility partners and referral sources across the community.
Qualifications
Required
- MD or DO from an accredited medical school.
- Board Certified or Board Eligible in:
- Internal Medicine
- Geriatric Medicine
- Eligible for Missouri medical licensure.
- DEA certification.
- Commitment to patient-centered, team-based care.
Preferred
- Fellowship training in Geriatric Medicine.
- Experience in Post-Acute Care, Skilled Nursing, Long-Term Care, Hospital Medicine, or Transitional Care.
- Interest in value-based care and population health management.
