SUMMARY

As a Nurse Practitioner in Geriatrics, you ll provide comprehensive primary and acute care to older adults across both Long-Term Care and Assisted Living settings. This role offers the opportunity to build meaningful relationships with patients while managing a diverse range of acute and chronic conditions. You ll work alongside a supportive team with strong onboarding and collaboration with nursing, respiratory therapy, facility staff, and experienced geriatric physicians.

ATTRIBUTES

  • Prior experience as a provider is preferred

RESPONSIBILITIES

  • Patient Care. Provide primary and acute care for geriatric patients, including assessment and management of acute and chronic conditions, physical examinations, diagnosis, treatment planning, medication management, and follow-up care

  • Long-Term Care. Provide care 2 3 days per week at Unity Living Center, including management of long-term care residents and patients requiring long-term ventilator support in collaboration with respiratory therapy

  • Assisted Living. Provide primary care 2 3 days per week to residents in assisted living settings, working closely with facility staff and the Geriatric Medical Director

  • Care Coordination. Collaborate with nursing, respiratory therapy, facility staff, physicians, and other members of the care team to coordinate comprehensive patient care

  • Documentation. Maintain accurate medical records, complete required documentation, and monitor patient response to treatment
