Cleveland Clinic Center for Connected Care is seeking a physician to join our growing home-based primary care practice. Join an innovative home care team that combines advanced technology and new care models with old fashioned compassionate, patient-centered care to better serve older adults with chronic conditions, while in the comfort of their homes.

This opportunity is currently available on the west side of our geographic territory. The practice will focus primarily on patients in assisted or independent living settings.

Clinical responsibilities may vary based on physician expertise, interests, and organizational needs. Duties may include in-home primary care, geriatric assessment and consultation, supervision of advanced practice nurses, and medical oversight of home care and hospice services. Additional opportunities may include leadership, research, clinical program development, medical education, and service integration across Cleveland Clinic Health System and community facilities. Weekend call is shared at 1:6 and is primarily telephone/home call. Salary is competitive and commensurate with experience.

Opportunities are available to split time with an office-based practice, skilled nursing facility practice, virtual medicine role, or inpatient care setting. Part-time candidates and flexible schedules are welcome.

Candidates must be board certified or board eligible in Internal Medicine or Family Medicine. A special interest and/or certification in Geriatric Medicine is strongly preferred; training is available.
